Job Description
Deliver essential care directly to patients homes by utilizing your advanced clinical skills for Medicare Advantage plan members across Union County, NJincluding Elizabeth, Plainfield, and Linden. As a per diem Nurse Practitioner in home health, you will perform health risk assessments during convenient, flexible shifts, with each encounter averaging approximately 45 minutes. The service provided may include annual wellness exams or targeted post-discharge evaluations.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete comprehensive annual home health risk assessments for Medicare Advantage members
- Thoroughly review and evaluate prior and current medical history, health conditions, and medication regimens
- Deliver patient education on medical conditions and proper medication usage
- Identify care gaps and recommend appropriate next steps for improved health outcomes
- No on-site treatment plan development or prescribing required
Qualifications:
- Active Nurse Practitioner license in the state of NJ
- Current CPR (BLS) certification
- Board certified via AANP or ANCC
- Minimum 6 months recent experience working as a Nurse Practitioner preferred
- MSN degree from an accredited nursing school
