Job Description

Step into a vital role supporting patient care in the community by performing home health risk assessments for Medicare Advantage members throughout Union County, NJ. This per diem opportunity offers Nurse Practitioners the flexibility to craft their own schedulechoose shifts any day of the week that suit your availability, with the option for weekly or bi-weekly minimum hourly commitments.

Responsibilities:

  • Conduct annual and post-discharge home health risk assessments for Medicare Advantage plan members in Union County communities such as Elizabeth, Plainfield, and Linden
  • Evaluate patients medical history, present conditions, and medication records in home settings
  • Educate patients about their conditions and medications to empower better health outcomes
  • Identify care gaps and offer actionable recommendations to enhance quality of careno requirement for treatment planning or medication prescribing
  • Navigate between various patient locations in Union County during your scheduled shifts

Qualifications:

  • Active Nurse Practitioner license in New Jersey
  • MSN degree from an accredited nursing program
  • Board certification from AANP or ANCC
  • Current CPR (BLS) certification
  • At least 6 months of current experience as a Nurse Practitioner preferred

Compensation & Perks: This role provides compensation of $80$90 per completed evaluation, with an added mileage stipend and payment for administrative time.
