Serve a diverse community as a Nurse Practitioner specializing in home health risk assessments for Medicare Advantage members across Middlesex County, NJ. This per diem role is ideal for healthcare professionals seeking flexibility and autonomy, with shifts available in Edison, Woodbridge, Old Bridge, and nearby areas.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct comprehensive annual home health risk assessments in patient homes
- Review and evaluate prior and current medical history, ongoing conditions, and medication regimens
- Offer education regarding medical conditions, medications, and preventive care
- Identify care gaps and provide recommendations to enhance patient outcomes
- Visits may include annual health assessments or post-discharge evaluations
- No requirement to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in the state of New Jersey
- Current Basic Life Support (BLS) CPR certification
- Board certification through AANP or ANCC
- A minimum of six months experience as an NP is preferred
- MSN from an accredited nursing program
