Step into a rewarding per diem role providing essential home health risk assessments for Medicare Advantage plan members throughout Middlesex County, NJ. As a Nurse Practitioner, you will conduct thorough evaluations in patients homes, contributing crucially to preventative care, health education, and improved patient outcomes. Each assessment typically requires about 45 minutes, with visits ranging from annual health evaluations to post-discharge follow-ups. Locations may include Edison, Woodbridge, Old Bridge, and additional areas within Middlesex County.

Schedule:

• Flexible shifts available seven days a week (Monday through Sunday)
• Tailor your schedule to your needs
• Minimum weekly or bi-weekly hourly commitment to fit your availability

Key Responsibilities:

• Complete comprehensive home health risk assessments for Medicare Advantage members
• Review prior and current medical histories, health conditions, and medication regimens
• Offer patient education related to medical conditions and medications
• Identify and address gaps in care, sharing actionable recommendations
• No requirement to develop treatment plans or prescribe medications

Qualifications:

• Current Nurse Practitioner license valid in New Jersey
• Active CPR certification (BLS)
• Board Certification via AANP or ANCC
• MSN degree from an accredited institution
• Minimum of six months recent experience as a Nurse Practitioner preferred
