Discover a unique opportunity to provide essential care in a flexible, per diem role as a Nurse Practitioner specializing in home health assessments. This position focuses on conducting risk assessments for Medicare Advantage plan members across Washington County, including areas such as Plymouth, Roper, and Creswell. Each assessment visit averages around 45 minutes and may include annual evaluations or post-discharge check-ins.
Schedule:
- Flexible shifts available any day, Monday through Sunday
- Design your own work schedule
- Choose your weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage members in their homes
- Review and document patients medical histories, current health status, and medications
- Educate patients about their conditions and prescribed medications, ensuring understanding and compliance
- Identify any gaps in care and recommend steps for improved patient outcomes
- Note: No treatment planning or prescribing of medications is required
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Up-to-date CPR certification (BLS)
- National certification through either AANP or ANCC
- At least 6 months of current Nurse Practitioner experience preferred
- MSN degree from an accredited nursing program
