Job Description

Enjoy the flexibility of per diem work while making a meaningful impact through comprehensive home health assessments. As a Nurse Practitioner specializing in home health, you will conduct risk assessments for Medicare Advantage plan members in Washington County, NC. These visits, lasting approximately 45 minutes each, may range from annual health assessments to post-discharge evaluations. Your expertise will ensure quality care for members residing in areas such as Plymouth, Roper, and Creswell, with assignments varying throughout Washington County during individual shifts.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Ability to create your own schedule
  • Minimum commitment required on a weekly or bi-weekly basis

Key Responsibilities:

  • Perform in-home health risk assessments for Medicare Advantage members
  • Review and evaluate prior and current medical history, conditions, and medication usage
  • Educate members about their health conditions and prescribed medications
  • Detect gaps in care and provide recommendations to enhance health outcomes
  • No requirement to develop treatment plans or prescribe medications

Qualifications:

  • Valid Nurse Practitioner license in North Carolina
  • Current CPR certification (BLS)
  • Board Certification through AANP or ANCC
  • At least 6 months of current experience as an NP preferred
  • MSN degree from an accredited nursing program

Compensation: Earn $80-$90 per completed evaluation, plus a mileage stipend and reimbursement for administrative time.
