Advance your nursing career providing essential home health services in a dynamic, flexible per diem role. As a Nurse Practitioner specializing in home health, you will play a crucial part in improving patient outcomes by conducting risk assessments for Medicare Advantage plan members throughout Washington County, NCserving communities such as Plymouth, Roper, and Creswell.
Schedule:
- Flexible shifts available Monday through Sunday
- Freedom to create your own schedule
- Minimum commitment required weekly or bi-weekly
What Youll Do:
- Conduct comprehensive annual home health risk assessments during in-home visits (approximately 45 minutes per assessment)
- Evaluate patients current and prior health histories, ongoing conditions, and medication regimens
- Provide patient education regarding disease management, medications, and proactive health measures
- Identify care gaps and offer actionable recommendations for enhancing patient well-being
- No requirement for prescribing medications or developing treatment plans
Desired Qualifications:
- Active Nurse Practitioner license in North Carolina
- Current CPR certification (BLS)
- Board certification through AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited school
- At least six months of recent NP experience preferred
This role offers compensation of $80 to $90 per evaluation, with an additional mileage stipend and administrative time provided, ensuring your expertise is valued.
