Job Description

Deliver high-quality care as a Nurse Practitioner specializing in Home Health risk assessments for Medicare Advantage members in Edenton and across Chowan County. Enjoy the flexibility of per diem work with the ability to design your own schedule and fulfill vital healthcare needs in communities like Tyner, Ryland, and Valhalla.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Set your own hours and work days
  • Choose your weekly or bi-weekly workload

Key Responsibilities:

  • Perform comprehensive home health risk assessments, typically 45 minutes each
  • Review and evaluate patients medical histories, conditions, and medications
  • Educate members about their health status, medications, and ongoing wellness practices
  • Identify care gaps and offer actionable recommendations; no treatment or prescriptions necessary
  • Complete required documentation post-visit and participate in administrative reporting as needed

Qualifications:

  • Active Nurse Practitioner license in North Carolina
  • Current CPR certification (BLS)
  • Board Certified through AANP or ANCC
  • Minimum of six months experience as a Nurse Practitioner preferred
  • Masters degree in Nursing (MSN) from an accredited institution

This position offers compensation ranging from $80 to $90 per evaluation, along with a mileage stipend and pay for administrative tasks recognizing both your clinical expertise and time spent on the road.
