This per diem role offers you the ability to create your own schedule, with shifts available any day of the week, making it ideal for practitioners seeking autonomy and impactful patient interactions within their community.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule
- Commit to a minimum weekly or bi-weekly hourly arrangement
Key Responsibilities:
- Perform comprehensive home health risk assessments, spending approximately 45 minutes per visit with Medicare Advantage members
- Assess and review prior/current medical history, chronic conditions, and medication usage
- Deliver patient education regarding medical conditions and medication regimens
- Identify care gaps and recommend actions to address them
- Visits may include annual wellness reviews or post-discharge evaluations; no requirement to prescribe medications or create treatment plans
Qualifications:
- Active Nurse Practitioner License in North Carolina
- Current CPR certification (BLS)
- Board Certification from AANP or ANCC
- Masters degree in Nursing (MSN) from an accredited institution
- At least 6 months of recent NP experience is preferred
Compensation for this role ranges from $80 to $90 per completed evaluation, with a mileage stipend and compensation for administrative time.
