Step into a rewarding opportunity as a Nurse Practitioner providing home health risk assessments within Wapello County. This flexible, part-time position centers on conducting comprehensive evaluations for Medicare Advantage members, focusing on annual assessments and post-discharge visits. Each assessment typically requires about 45 minutes, and visit locations may vary across communities, including Agency, Blakesburg, Chillicothe, Eldon, and Kirkville.

Schedule:

• Flexible shifts available Monday through Sunday
• Design your own schedule to fit your lifestyle
• Minimal weekly or bi-weekly hourly commitment

Key Responsibilities:

• Complete thorough home health risk assessments for Medicare Advantage plan members
• Review and evaluate past and current medical and medication histories
• Offer education related to chronic health conditions and medications
• Identify and report care gaps, recommending steps to improve patient outcomes
• Please note: No treatment planning or medication prescribing is involved

Qualifications & Experience:

• Valid Nurse Practitioner license in Iowa
• MSN degree from an accredited nursing program
• Board Certification (AANP or ANCC)
• Current CPR certification (BLS)
• At least one year of Nurse Practitioner experience preferred, though new practitioners are welcome to apply
