Job Description

A skilled and dedicated Nurse Practitioner is sought for a per diem role conducting in-home health risk assessments for Medicare Advantage plan members across Preble County. You will have the flexibility to create your own schedule with shifts available between Monday and Sunday and a required minimum commitment weekly or bi-weekly.

Key Responsibilities:

  • Perform annual home health risk assessments for Medicare Advantage members in homes throughout the area
  • Evaluate prior and current medical history, existing conditions, and medication history
  • Educate members on medical conditions and the proper use of medications
  • Identify gaps in care and offer recommendations for improvement
  • No requirement for the creation of treatment plans or prescribing medications

Desired Qualifications:

  • Active Nurse Practitioner License in Ohio
  • Board Certification via AANP or ANCC required
  • Current CPR certification (BLS)
  • MSN degree from an accredited nursing program
  • At least 6 months of recent NP experience preferred
