Job Description
Deliver compassionate care directly to patients homes by conducting home health risk assessments for Medicare Advantage members throughout Preble County, Ohio. As an experienced Nurse Practitioner, you will focus on comprehensive evaluations that provide valuable insight into a patients health status, without the need for ongoing treatment planning or prescribing. This per diem role allows you to make a meaningful impact on patient health, all while enjoying the flexibility to design your own schedule.
Schedule:
- Flexible shifts offered Monday through Sunday
- Set your own workdays and hours
- Choose your level of commitment minimum weekly or bi-weekly shifts available
Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage participants
- Review patients prior and current medical histories, existing conditions, and medication use
- Educate individuals on their medical conditions and prescribed medications
- Identify gaps in care and recommend solutions to address those needs
- Conduct visits in various areas within Preble County as assigned per shift
- No requirements for creating treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner License in Ohio
- Board Certification from AANP or ANCC
- Valid BLS/CPR certification
- MSN degree earned at an accredited nursing program
- At least 6 months of nurse practitioner experience (strongly preferred)
Your dedication will be rewarded with competitive pay between $80 and $90 per evaluation, along with a mileage stipend and compensation for administrative time.
