Deliver exceptional care with the flexibility you desire as a Nurse Practitioner specializing in home health services. This per diem position focuses on conducting home health risk assessments for Medicare Advantage plan members throughout Madison County, Illinoisincluding areas such as Granite City, Edwardsville, and Alton.
Schedule:
- Flexible shifts available any day of the week, Monday through Sunday
- Design your own schedule to suit your availability
- Minimum weekly or bi-weekly hourly commitment as preferred
Key Responsibilities:
- Perform comprehensive annual and post-discharge risk assessments in patient homes
- Review prior and current medical histories, assess medical conditions, and evaluate medication usage
- Educate patients on health conditions, treatment expectations, and medication adherence
- Recognize care gaps and recommend appropriate steps for follow-up or resolution
- Focus on assessmentno treatment plan creation or medication prescribing required
Qualifications:
- Active Nurse Practitioner license in Illinois
- CPR certification (BLS required)
- Board certification through AANP or ANCC
- At least 6 months of recent Nurse Practitioner experience preferred
- MSN degree from an accredited nursing school
In this role, you will complete each evaluation within an average of 45 minutes, visiting patients in various locations throughout Madison County. Compensation for your expertise includes $80$90 for each completed assessment, with a mileage stipend and administrative time also covered.
