Make a direct impact on patient care by providing comprehensive home health risk assessments for Medicare Advantage plan members throughout St. Clair County, Illinois, including areas such as Belleville, O'Fallon, and East St. Louis. This per diem position is ideal for Nurse Practitioners seeking autonomy and flexibility, while enhancing the lives of individuals in their community.
Schedule:
- Flexible shifts available any day, Monday through Sunday

- Minimum hourly commitment on a weekly or bi-weekly basis
Key Responsibilities:
- Perform thorough annual home health risk assessments in patients homes
- Review medical history, current medical conditions, and medication regimens
- Deliver patient education regarding health conditions and prescribed medications
- Identify gaps in care and offer actionable recommendations for follow-up
- No requirement to create treatment plans or prescribe medications
Qualifications:
- Current and unencumbered Nurse Practitioner License in Illinois
- Valid CPR certification (BLS)
- Board certification through AANP or ANCC
- MSN from an accredited nursing program
- At least 6 months of recent Nurse Practitioner experience (preferred)
This role offers competitive compensation of $80$90 per completed evaluation, a mileage stipend to cover travel between patient locations, and compensation for administrative time.
