Job Description

This per diem role is designed for practitioners who seek flexibility and meaningful patient connectionsserving communities in Edwardsville, Alton, Granite City, and neighboring areas. Create your own schedule, with flexible shifts available every day of the week.

Schedule:

  • Flexible shifts Monday through Sunday
  • Arrange your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Perform comprehensive annual home health risk assessments in patient homes
  • Review and evaluate prior and current medical history, conditions, and medications
  • Educate patients on health conditions and medication management
  • Identify care gaps and provide actionable recommendations for improved outcomes
  • No requirement for developing treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner License in Illinois
  • Board Certification through AANP or ANCC
  • Current BLS (CPR) certification
  • MSN from an accredited institution
  • A minimum of 6 months current NP experience preferred

This role offers compensation of $80-$90 per completed evaluation, supplemented by a mileage stipend and payment for administrative time.
