Key Responsibilities:

  • Perform comprehensive home health risk assessments for Medicare Advantage plan members
  • Review and evaluate prior and current medical history, chronic and acute medical conditions, and medication usage
  • Deliver patient-focused education regarding medical conditions and medication compliance
  • Identify care gaps and provide action-oriented recommendations to improve patient health outcomes
  • Document all findings accurately; note that treatment plan development and medication prescribing are not required duties

Qualifications:

  • Active Nurse Practitioner License in Illinois
  • Valid CPR certification (BLS)
  • Board certification through AANP or ANCC
  • MSN degree from an accredited nursing program
  • Minimum of 6 months recent Nurse Practitioner experience preferred
