This per diem role offers flexible scheduling, allowing you to design your own shifts between Monday and Sunday, with minimum weekly or bi-weekly hourly commitments to fit your lifestyle. Key Responsibilities:
- Perform comprehensive home health risk assessments for Medicare Advantage plan members
- Review and evaluate prior and current medical history, chronic and acute medical conditions, and medication usage
- Deliver patient-focused education regarding medical conditions and medication compliance
- Identify care gaps and provide action-oriented recommendations to improve patient health outcomes
- Document all findings accurately; note that treatment plan development and medication prescribing are not required duties
Qualifications:
- Active Nurse Practitioner License in Illinois
- Valid CPR certification (BLS)
- Board certification through AANP or ANCC
- MSN degree from an accredited nursing program
- Minimum of 6 months recent Nurse Practitioner experience preferred
