In this per diem role, you'll provide essential health assessments for Medicare Advantage plan members across Transylvania County, including the Brevard and Rosman areas. This opportunity allows you to make a meaningful impact on patients lives while customizing your shifts to suit your lifestyle.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own weekly or bi-weekly schedule
- Choose shifts as your availability permits
Key Responsibilities:
- Conduct annual and post-discharge home health risk assessments for Medicare Advantage members in their residences
- Review and evaluate patients medical histories, current health status, and medication usage
- Offer clear education on health conditions and prescribed medications, empowering individuals to take charge of their wellness
- Identify care gaps and recommend steps for improvement
- Focus solely on assessmentsno requirement for prescribing medications or developing treatment plans
Qualifications:
- Active Nurse Practitioner License in North Carolina
- Board Certification via AANP or ANCC
- Current BLS (CPR) certification
- MSN degree from an accredited nursing program
- At least six months of nurse practitioner experience (preferred)
This position offers $80$90 per evaluation, with additional compensation for mileage and administrative responsibilities.
