In this per diem role, Nurse Practitioners will travel to residences within Wyoming County, NYincluding towns such as Warsaw, Perry, and Atticato provide essential evaluations for Medicare Advantage plan members. Each comprehensive home assessment averages 45 minutes and may focus on annual health evaluations or post-discharge visits. Your expertise helps identify health needs, provide patient education, and ensure care qualitywithout treatment planning or prescribing responsibilities.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Design your own work schedule
  • Choose your hours on a weekly or bi-weekly basis

Key Responsibilities:

  • Perform annual home health risk assessments for a Medicare Advantage health plan
  • Review medical and medication history and assess current medical conditions
  • Deliver education to patients about their conditions and medications
  • Identify gaps in care and recommend follow-up steps
  • No requirement to create treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in New York
  • Board Certification through AANP or ANCC
  • Current CPR (BLS) certification
  • MSN from an accredited institution
  • Minimum of 6 months recent Nurse Practitioner experience preferred
