Make a direct impact in patient care by conducting home health risk assessments for Medicare Advantage plan members across Jackson County. As a Nurse Practitioner specializing in home health, you'll work in diverse communities, including Ashland, Eagle Point, Jacksonville, Medford, and nearby areas. This per diem opportunity offers the autonomy to design your schedule, providing remarkable flexibility to fit your lifestyle.
Schedule:
- Flexible shifts available Monday through Sunday
- Customize your own schedule weekly or bi-weekly
- Select the level of time commitment that fits your availability
Key Responsibilities:
- Conduct thorough annual and post-discharge home health risk assessments in the patients residence
- Review and document prior/current medical conditions, medication history, and overall health status
- Offer personalized education about medical conditions and medication management
- Identify care gaps and provide actionable recommendations for optimal health outcomes
- No responsibility for creating treatment plans or prescribing medicationsfocus strictly on assessments and education
Qualifications:
- Active Nurse Practitioner license in Oregon
- Board certification through AANP or ANCC
- Current Basic Life Support (BLS/CPR) certification
- Minimum MSN degree from an accredited nursing school
- At least 6 months of experience as a Nurse Practitioner is preferred
You will receive compensation of $80$90 per completed evaluation, plus a mileage stipend and consideration for administrative work involved with each assessment. This unique per diem role suits practitioners seeking both patient-facing time and autonomy over their work-life balance.
