Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule
- Commitment available on a weekly or bi-weekly basis
Key Responsibilities:
- Conduct comprehensive annual home health risk assessments for Medicare Advantage health plan members
- Evaluate patients current and past medical histories, conditions, and medication use
- Educate patients regarding medical conditions and proper medication management
- Identify any care gaps and make recommendations for closing these gaps
- No treatment planning or prescribing responsibilities are requiredfocus is solely on assessment and education
Desired Qualifications:
- Active Nurse Practitioner license in the state of Tennessee
- Current CPR (BLS) certification
- Board certification through AANP or ANCC
- Minimum of 6 months recent experience as a Nurse Practitioner preferred
- Master of Science in Nursing (MSN) from an accredited program
Compensation:
- Earn $80$90 per evaluation, with additional mileage stipend and administrative time compensation
