Step into a vital per diem role conducting home health risk assessments for Medicare Advantage plan members. As an experienced nurse practitioner, youll play a key part in supporting the wellbeing of individuals across Durham County. This role offers true flexibilityset your own schedule with shifts available any day of the week, and a minimum weekly or bi-weekly hourly commitment to fit your needs.
Schedule:
- Flexible shifts offered Monday through Sunday
- You choose your own schedule
- Minimal required hourly commitment weekly or bi-weekly
Key Responsibilities:
- Perform comprehensive, 45-minute home health risk assessments for plan members in their residences
- Evaluate medical history, current conditions, and medications
- Educate members about health conditions and proper medication use
- Identify gaps in care and provide recommendations
- Complete assessments without developing treatment plans or prescribing medication
Assessments will take place throughout Durham County in locations that may include Raleigh, Morrisville, and Cary. You will be scheduled in various areas as needed, ensuring variety and dynamic patient engagement.
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Board Certification (AANP or ANCC)
- Current BLS (CPR) certification
- MSN degree from an accredited institution
- At least 6 months of current NP experience preferred
You will be compensated at $80-$90 per evaluation, with an additional mileage stipend and administrative time provided.
