Key Responsibilities:
- Perform comprehensive home health risk assessments (typically 45 minutes each) for Medicare Advantage plan members.
- Review and document patients medical histories, current conditions, and medication regimens.
- Educate patients on their health status and medication usage.
- Identify gaps in care and communicate actionable recommendations.
- Visits may encompass annual health assessments or post-discharge evaluations.
- No treatment planning or prescribing required.
Schedule/Hours:
- Flexible shifts available seven days a week.
- Create your own work schedule to suit your needs.
- Choose minimal weekly or bi-weekly hours.
Required Qualifications:
- Current Nurse Practitioner license in North Carolina.
- Board Certification through AANP or ANCC.
- CPR certification (BLS required).
- Master of Science in Nursing (MSN) from an accredited institution.
- Minimum 6 months of NP experience, preferably in home health settings.
