Job Description
Opportunity available for an experienced Nurse Practitioner to deliver high-quality home health risk assessments. This independent, per diem position allows you to work directly with Medicare Advantage members in Durham County, conducting comprehensive evaluations in the comfort of their own homes. Assessments may include annual health reviews or post-discharge visits, tailored to each members needs.
Schedule:
- Flexible shifts available Monday through Sunday

- Choose your minimum weekly or bi-weekly hourly commitment
Responsibilities:
- Travel to members residences within Durham Countyincluding the surrounding areasto conduct 45-minute risk assessment visits
- Perform thorough reviews of prior and current medical histories, conditions, and medication regimens
- Educate members on managing conditions and understanding their medications
- Identify any gaps in care, making recommendations for further follow-up
- Focus solely on assessmentno prescribing medications or creating treatment plans required
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Current Basic Life Support (BLS) certification
- Board Certification through AANP or ANCC
- At least six months of recent Nurse Practitioner experience (preferred)
- Master of Science in Nursing (MSN) from an accredited institution
