Deliver essential care and valuable insights as a Nurse Practitioner in Home Health by performing comprehensive risk assessments for Medicare Advantage members throughout Marion County, FL, including Ocala, Belleview, and Dunnellon. This per diem opportunity empowers you to make a tangible impact through patient-centered visits, assisting members in managing their health and identifying crucial gaps in care.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule to match your availability
- Choose your preferred minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct home health risk assessments, averaging 45 minutes per visit, for Medicare Advantage plan members
- Perform annual health assessments and post-discharge visits following specific protocols
- Evaluate and document comprehensive medical histories, current conditions, and medication regimens
- Deliver tailored education on chronic conditions and prescribed medications
- Detect gaps in care and recommend practical solutionswithout creating treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner License in Florida
- Board Certification via AANP or ANCC
- MSN degree from an accredited nursing school
- Current BLS CPR certification
- Minimum of 6 months current NP experience preferred
Your commitment to high-quality, community-based care will be rewarded with compensation of $80-$90 per evaluation, plus a mileage stipend and administrative pay. Youll practice autonomously, with flexibility to schedule visits within your preferred service areas around Marion County.
