Per diem opportunities are available for experienced nurse practitioners to conduct in-home visits with Medicare Advantage members, focusing on wellness and quality of care.

Core Responsibilities:
- Perform comprehensive annual health assessments or post-discharge visits in patients homes
- Evaluate patients prior and current medical, surgical, and medication history
- Identify gaps in care, offering recommendations to promote optimal health outcomes
- Deliver education to patients on diagnoses, medications, and health management
- Complete thorough documentation for each evaluation
- No requirement to prescribe or develop new treatment plans
Required Qualifications:
- Active Nurse Practitioner license in North Carolina
- Board Certification (AANP or ANCC)
- Valid and current BLS/CPR certification
- MSN from an accredited nursing program
- Minimum of 6 months experience working as an NP (preferred)
- Ability to travel within Harnett County, including areas such as Lillington, Anderson Creek, Spout Springs, and Dunn
This role offers per-evaluation earnings between $80 and $90, with additional mileage reimbursement and compensation for administrative time.
