Advance your career as a Nurse Practitioner in a flexible per diem role conducting home health risk assessments. In this position, you will make a significant impact by supporting the needs of Medicare Advantage plan members throughout Steuben County, including Bath, Corning, Gang Mills, and Hornell. Each home visit averages 45 minutes and may include an annual health assessment or a post-discharge evaluation.
Schedule:
- Flexible shifts, Monday through Sunday
- Self-scheduling for maximum autonomy
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete thorough annual risk assessments in members homes
- Review prior and current medical history, conditions, and medications
- Educate patients on health conditions and medication management
- Identify care gaps and provide clear recommendations to address them
- Provide comprehensive documentation; creating treatment plans and prescribing medication are not required
Qualifications:
- Active Nurse Practitioner license in New York State
- Master of Science in Nursing (MSN) from an accredited school
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- At least six months of recent experience as a Nurse Practitioner (preferred)
This role combines convenience and flexibility, with compensation of $80-$90 per completed in-home evaluation. Additionally, you will receive a mileage stipend and compensation for administrative work related to each assessment.
