Job Description

Schedule:

  • Flexible shifts available Monday through Sunday
  • Design your own work schedule
  • Minimum weekly or bi-weekly hourly commitment to match your availability

Core Responsibilities:

  • Conduct comprehensive home health risk assessments for Medicare Advantage members
  • Thoroughly review and evaluate previous and current medical histories, ongoing conditions, and medication use
  • Provide clear patient education regarding chronic conditions, preventative care, and medication management
  • Identify gaps in care and offer actionable recommendations to resolve them
  • Note: No prescribing of medications or development of treatment plans required

Required Qualifications:

  • Valid Nurse Practitioner license issued by the state of New York
  • Board certified by AANP or ANCC
  • Completion of a MSN degree from an accredited nursing program
  • Active CPR (BLS) certification
  • At least 6 months of current experience as a Nurse Practitioner is preferred
