Deliver high-quality patient care and make a difference in community health as a Nurse Practitioner specializing in home health risk assessments. In this per diem opportunity, you'll work closely with Medicare Advantage plan members in Mercer County. Assessments may be conducted in areas such as Charleston, South Charleston, and St. Albans. Your expertise will guide patients through essential health evaluations without the need to create treatment plans or prescribe medications.

• Commitment can be weekly or bi-weekly by hours

Key Responsibilities:

• Conduct thorough annual home health risk assessments for Medicare Advantage members in patient homes
• Evaluate patients current and previous medical history, ongoing medical conditions, and medication usage
• Offer educational support to patients about their conditions and medication regimens
• Identify gaps in care, providing recommendations for follow-up and resolution
• Focus strictly on comprehensive assessmentsno requirement to develop treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in West Virginia
• Board certified through AANP or ANCC
• MSN from an accredited nursing program
• Current BLS/CPR certification
• A minimum of 6 months of practical NP experience preferred
