Deliver comprehensive care and make a difference in the community through flexible, per diem home health assignments. As a Nurse Practitioner specializing in home health, you will conduct risk assessments and provide valuable insights for Medicare Advantage plan members across Desha County, including Dumas, McGehee, and Arkansas City. This role offers autonomy, flexibility, and the chance to build your ideal work schedule.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own schedule
- Minimum hourly commitment on a weekly or bi-weekly basis
Key Responsibilities:
- Conduct annual home health risk assessments for Medicare Advantage plan members in patients homes
- Review prior and current medical history, conditions, and medication use
- Educate members on health conditions and medication therapy
- Identify gaps in care and offer evidence-based recommendations to resolve them
- Visits may include annual assessments or post-discharge check-ins
- No prescribing medications or care plan creation required
Qualifications:
- Active Nurse Practitioner license in Arkansas
- Master of Science in Nursing (MSN) from an accredited program
- Board Certification through AANP or ANCC
- Current BLS (CPR) certification
- At least 6 months of recent NP experience preferred
