Step into an impactful Nurse Practitioner role providing home health risk assessments for Medicare Advantage plan members throughout Desha County, Arkansas. This per diem opportunity empowers you to create your own schedule with the flexibility to work shifts between Monday and Sunday and the option to commit to a minimum hourly requirement weekly or bi-weekly.
Key Responsibilities:
- Perform comprehensive risk assessments in members homes, averaging 45 minutes per evaluation
- Review current medical conditions, prior medical history, and medication lists
- Educate members on their health conditions and medications, supporting their ability to manage wellness at home
- Identify gaps in care and recommend next steps for improved health outcomes
- Focus visits on evaluationno treatment plans or medication prescriptions required
Schedule:
- Flexible scheduling available Monday through Sunday
- Design your work schedule to fit your lifestyle
- Minimum hourly commitment per week or every other week
Qualifications:
- Active and unrestricted Nurse Practitioner license in the state of Arkansas
- Board Certified through either the AANP or ANCC
- Current CPR certification (BLS)
- MSN degree from an accredited nursing program
- At least 6 months of experience as a Nurse Practitioner preferred
