Make a real impact by delivering essential care directly to patients homes as a Nurse Practitioner specializing in home health risk assessments. This per diem opportunity focuses on providing high-quality evaluations for Medicare Advantage members throughout Pulaski County, giving you the flexibility and autonomy to create your ideal work schedule.
Schedule:
- Flexible shifts available Monday through Sunday
- Enjoy the freedom to design your own schedule
- Meet a minimum hourly commitment on a weekly or bi-weekly basis
Key Responsibilities:
- Conduct comprehensive annual home health risk assessments for Medicare Advantage participants
- Review and document prior and current medical history, existing medical conditions, and full medication profiles
- Offer clear, compassionate education to patients regarding their health conditions and medications
- Identify gaps in care, addressing them with thoughtful recommendations
- No requirement to develop treatment plans or prescribe medication
Qualifications:
- Active Nurse Practitioner licensure in Virginia
- Board Certification from AANP or ANCC
- CPR certification (BLS)
- Master of Science in Nursing (MSN) from an accredited institution
- At least 6 months of recent NP experience preferred
