Job Description
Make a meaningful impact in patients lives by providing crucial home health risk assessments to Medicare Advantage plan members in Kent County. In this per diem opportunity, your clinical expertise as a Nurse Practitioner will directly improve health outcomes through comprehensive annual and post-discharge evaluations, conducted at patients homes throughout Dover, Smyrna, and Milford.
Schedule/Hours
- Flexible shifts available Monday through Sunday
- Self-schedulingset your own hours
- Option for minimum weekly or bi-weekly hourly commitment
Key Responsibilities
- Complete detailed annual home health risk assessments for Medicare Advantage members
- Gather and review prior and current medical history, chronic conditions, and medication use
- Deliver individualized education regarding disease management and medication safety
- Proactively detect gaps in care, documenting recommendations to address patient needs
- No involvement in treatment planning or medication prescribing
Qualifications
- Active Nurse Practitioner license in Delaware
- Board Certification through AANP or ANCC
- Current CPR (BLS) certification
- MSN degree from an accredited nursing program
- At least 6 months of recent NP experience preferred
