Exciting opportunity for an experienced Nurse Practitioner to perform home health risk assessments for Medicare Advantage plan members in Douglas County. This per diem role lets you make a significant impact on patient care and education in the comfort of each members residence.

Schedule:

• Flexible shifts available Monday through Sunday
• Design your own work schedule
• Minimum weekly or bi-weekly hour commitment required

Key Responsibilities:

• Conduct comprehensive annual risk assessments for Medicare Advantage members in their homes
• Evaluate both past and current medical histories, active medical conditions, and medication regimens
• Educate patients on their health conditions and medication use
• Identify care gaps and provide informed recommendations to improve patient outcomes
• No responsibility for creating treatment plans or prescribing medications

Qualifications:

• Nurse Practitioner license valid in Georgia
• Active BLS/CPR certification
• Board certified through AANP or ANCC
• Minimum six months recent experience as a Nurse Practitioner preferred
• MSN from an accredited nursing program

This position covers Douglasville, Villa Rica, Austell, and surrounding Douglas County area members. Candidates may be scheduled at any location within the county during their shifts. Enjoy earning $80-$90 per completed evaluation, plus mileage reimbursement and paid administrative time.
