Job Description

Make a meaningful impact in the community by serving as a Nurse Practitioner specializing in home health risk assessments for Medicare Advantage plan members. This per diem role offers the chance to support patients in Douglasville and surrounding areas of Douglas County, including Villa Rica and Austell, ensuring they receive comprehensive evaluations from the comfort of their homes.

Schedule:

Key Responsibilities:

  • Conduct annual home health risk assessments for Medicare Advantage members during 45-minute visits, which may include annual check-ups or post-discharge assessments
  • Review both past and present medical history, evaluate ongoing conditions, and assess medication regimens
  • Educate patients about their health conditions and prescribed medications
  • Identify gaps in care and provide actionable recommendations to improve patient outcomes
  • Focus exclusively on assessmentsthere is no requirement to create treatment plans or prescribe medications

Qualifications:

  • Active and unrestricted Nurse Practitioner License in Georgia
  • Board Certification through AANP or ANCC
  • Current BLS/CPR certification
  • MSN degree from an accredited nursing program
  • Minimum of 6 months of current NP experience preferred
