Job Description

Enjoy the flexibility of working per diem as a Nurse Practitioner specializing in home health risk assessments. In this role, you will visit Medicare Advantage plan members in their residences throughout Essex County, Massachusetts, including Lynn, Lawrence, and Haverhill. These visits, which typically last around 45 minutes, may include annual health evaluations or post-discharge assessments, offering meaningful interactions directly in the community.

Key Responsibilities:

  • Conduct comprehensive home health risk assessments for Medicare Advantage members
  • Review and document medical history, current health conditions, and medication use
  • Educate patients on their medical conditions and appropriate medication usage
  • Identify and communicate any gaps in care, providing recommendations to support patient well-being
  • Note: This position does not involve developing treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in Massachusetts
  • Current CPR certification (BLS)
  • Board certification through AANP or ANCC
  • Minimum of 6 months' recent NP experience (preferred)
  • Master of Science in Nursing (MSN) from an accredited institution

In addition to meaningful patient interactions and autonomous practice, you will be compensated at $80-$90 per completed evaluation, with additional stipends for mileage and administrative time.
