Key Responsibilities:

• Conduct comprehensive annual home health risk assessments during 45-minute visits
• Evaluate patients medical history, current conditions, and medication regimens
• Educate patients on their health conditions and prescribed medications
• Identify gaps in care and provide recommendations for improved outcomes
• Focus solely on assessmentsno prescribing medications or formulating treatment plans required

Qualifications:

• Active Nurse Practitioner license in Missouri
• MSN degree from an accredited nursing program
• Board Certification through AANP or ANCC
• Current CPR certification (BLS)
• At least 6 months of current NP experience is preferred

Schedule:

• Flexible hours with shifts available between Monday and Sunday
• Design your own work schedule
• Minimum weekly or bi-weekly hourly commitment
