Embark on a rewarding per diem opportunity as a Nurse Practitioner providing home health risk assessments throughout Surry County, North Carolina. This flexible role enables you to optimize your schedule while delivering crucial healthcare services to Medicare Advantage plan members in Mount Airy, Elkin, Dobson, and neighboring areas.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule to accommodate your lifestyle
- Minimum hourly commitment required weekly or bi-weekly
Responsibilities:
- Perform comprehensive home health risk assessments for Medicare Advantage health plan members
- Review prior and current medical history, conditions, and medication regimens
- Educate patients on relevant health conditions and proper medication usage
- Identify care gaps and deliver actionable recommendations for improvement
- Focus on assessment and educationno treatment planning or medication prescribing required
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Board Certification through AANP or ANCC
- Current BLS (CPR) certification
- MSN degree from an accredited nursing school
- At least 6 months of current NP experience preferred
