Job Description

Deliver essential home-based care as a Nurse Practitioner providing risk assessments for Medicare Advantage plan members across Lamar County, TX, including the communities of Paris, Reno, Powderly, Roxton, and Blossom. As a vital part of this per diem opportunity, you'll conduct comprehensive evaluations that guide patients in managing their health and closing vital care gapsall in the comfort of their homes.

Schedule:

  • Flexible scheduling available Monday through Sunday
  • Create your own shift times to accommodate personal and professional obligations
  • Minimum commitment required on a weekly or bi-weekly basis

Key Responsibilities:

  • Perform annual home health risk assessments for Medicare Advantage members, with each visit lasting approximately 45 minutes
  • Review prior and current medical history, chronic and acute health conditions, and medication use during each assessment
  • Deliver clear education to patients regarding their diagnoses and medication regimens
  • Recognize and highlight potential gaps in care, providing actionable recommendations for follow-up
  • Focus solely on risk assessmenttreatment plan creation or medication prescriptions are not part of this role

Qualifications:

  • Active Nurse Practitioner license in Texas
  • Current Basic Life Support (CPR) certification
  • Board Certification through AANP or ANCC required
  • Minimum of six months NP experience preferred
  • MSN degree from an accredited nursing program
