Nurse Practitioners skilled in home health are invited to take on a dynamic per diem opportunity conducting risk assessments for Medicare Advantage plan members in Polk County. This role is ideal for clinicians seeking flexibility and autonomy, with the ability to create your own schedule and work shifts that fit your lifestyle. Assessments are completed directly in members' homes located throughout Polk County, including areas like Urbandale, Johnston, and Clive.

Schedule:

• Flexible shifts available Monday through Sunday
• Set your own schedule
• Commit to a minimum number of hours weekly or bi-weekly

Key Responsibilities:

• Complete comprehensive annual home health risk assessments for Medicare Advantage members
• Review and document prior and current medical history, health conditions, and medication usage
• Provide patient education on chronic conditions, medications, and general wellness
• Identify any existing gaps in care and recommend next steps for resolution
• No prescribing of medications or creation of treatment plans is required

Qualifications:

• Active Nurse Practitioner license in Iowa
• Board certification through AANP or ANCC
• Current CPR (BLS) certification
• MSN degree from an accredited nursing program
• At least 6 months of recent Nurse Practitioner experience preferred

Clinicians performing assessments will earn $80-$90 per evaluation, with additional compensation for mileage and administrative time.
