Advance your Nurse Practitioner career with an engaging per-diem opportunity conducting essential home health risk assessments. As a provider, you will visit Medicare Advantage plan members throughout Polk County, including Urbandale, Johnston, and Clive. Each evaluation typically takes about 45 minutes and may include annual health assessments or post-discharge visits. Your work supports high-quality patient outcomes by identifying care gaps and promoting wellnessall without the need for developing treatment plans or prescribing medications.

Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage members
- Review and evaluate prior and current medical history, medical conditions, and medication usage
- Educate patients regarding their health conditions and medications
- Recognize and report care gaps, offering recommendations for improvement
- Focus on evaluationtreatment planning and prescribing are not required
Qualifications:
- Current Iowa Nurse Practitioner License
- Active CPR certification (BLS)
- Board Certification through AANP or ANCC required
- Minimum of 6 months recent Nurse Practitioner experience preferred
- MSN degree from an accredited nursing program
