Seeking a dynamic Nurse Practitioner for per diem home health risk assessments within Jefferson County, CO, including areas such as Lakewood, Arvada, and Westminster. This opportunity offers flexible scheduling and the autonomy to create your own work hours, making it an ideal fit for those looking to balance work with other commitments.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own schedule based on availability
- Minimum weekly or bi-weekly hourly commitments
Responsibilities:
- Conduct thorough annual home health risk assessments for Medicare Advantage members in their homes
- Review prior and current medical history, medical conditions, and medication use
- Deliver education to members regarding their health conditions and medications
- Identify gaps in care and provide actionable recommendations
- Focus on assessment and educationno need to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Colorado
- Current BLS certification (CPR)
- Board Certification through AANP or ANCC
- MSN degree from an accredited nursing school
- Minimum of six months experience as a Nurse Practitioner preferred
This role provides compensation of $80 to $90 per evaluation, including a mileage stipend and coverage for administrative time.
