Job Description
Seeking a Nurse Practitioner skilled in home health to provide comprehensive in-home risk assessments for Medicare Advantage members across Adams County, CO. This per diem opportunity offers flexible schedulingtailoring your work to fit your lifestyle with shifts available any day of the week, including weekends. Youll enjoy autonomy in managing your calendar with a minimum weekly or bi-weekly hourly commitment.
Schedule:
- Flexible shifts Monday through Sunday
- Set your own availability
- Minimum required hours on a weekly or bi-weekly basis
In this role, you will:
- Perform thorough annual risk assessments for Medicare Advantage plan members in their homes
- Review prior and current medical history, existing conditions, and medication use
- Deliver valuable patient education regarding health conditions and medications
- Detect gaps in care and provide actionable recommendations
- Complete post-discharge visits as needed
- No requirements for prescribing medications or developing long-term treatment plans
Qualifications:
- Active Nurse Practitioner license issued by the state of Colorado
- Board Certification through AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited institution
- Current CPR (BLS) certification
- At least 6 months of current NP experience preferred
You will receive compensation of $80-$90 for each completed evaluation along with a mileage stipend and administrative time included.
