Key Responsibilities:
- Perform focused home health risk assessments, typically 45 minutes per visit
- Assess patient medical history, current conditions, and medication use
- Deliver patient education regarding diagnoses and medications
- Identify gaps in care and recommend solutions to resolve them
- Visits are non-treatment focused: no prescribing or treatment planning required
Qualifications:
- Active Nurse Practitioner license in SC
- Board Certification: AANP or ANCC
- Master's degree (MSN) from an accredited nursing program
- Current CPR certification (BLS)
- At least 6 months of recent nurse practitioner experience preferred
