Experienced Nurse Practitioners are needed for per diem home health risk assessments with a leading Medicare Advantage health plan. This flexible position allows you to tailor your own schedule, serving members throughout Luna County, including Deming, Columbus, and City of the Sun. Home health visits include annual risk assessments and post-discharge assessments, each appointment averaging about 45 minutes. Your role will help ensure comprehensive care planning and health management for older adults in their own homes.

Schedule:

• Flexible shifts, MondaySunday
• Create your own schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct comprehensive home health risk assessments for Medicare Advantage members
• Evaluate current/prior medical history, active conditions, and medication lists
• Educate members on health conditions and medication regimens
• Identify gaps in care and provide actionable recommendations
• No requirement to prescribe medications or create treatment plans

Qualifications:

• Active Nurse Practitioner license in New Mexico
• Board Certification through AANP or ANCC
• Current CPR certification (BLS)
• Masters degree in Nursing (MSN) from an accredited program
• Minimum of 6 months NP experience preferred

Providers will receive competitive pay on a per-evaluation basis, inclusive of a mileage stipend and administrative time.
