Job Description
Make a significant impact on patient care as a Nurse Practitioner providing home health risk assessments. This per diem opportunity offers autonomy and flexibility while allowing you to reach individuals in need of comprehensive healthcare support in their homes.
Schedule:
- Flexible shifts available Monday through Sunday
- Build your own schedule for optimal work-life balance
- Commit minimum weekly or bi-weekly hours based on your availability
Key Responsibilities:
- Perform home health risk assessments for Medicare Advantage plan members
- Conduct thorough evaluations, reviewing prior and current medical history, health conditions, and medications
- Deliver clear patient education regarding medical conditions and the use of prescribed medications
- Identify gaps in care and provide actionable recommendations to improve health outcomes
- Visits may entail annual assessments or post-discharge follow-ups (approximately 45 minutes each)
- No requirement to create treatment plans or prescribe medication
Qualifications:
- Active Nurse Practitioner license in Florida
- MSN from an accredited nursing program
- Board certified by AANP or ANCC
- Current BLS (CPR) certification
- At least six months of recent experience as a Nurse Practitioner preferred
This per diem role provides competitive compensation of $80-$90 per evaluation, plus a mileage stipend and paid administrative time. Youll serve Medicare Advantage plan members throughout Sanford and neighboring areas in Seminole County.
