Schedule:

  • Flexible schedulingchoose your own shifts between Monday and Sunday
  • Set your preferred weekly or bi-weekly hour commitments
  • Assignments may include Deltona, Daytona Beach, Port Orange, or nearby areas within Volusia County

Key Responsibilities:

  • Complete comprehensive annual home health risk assessments for Medicare Advantage plan members
  • Assess complete medical histories, current conditions, and medication usage
  • Educate individuals on their chronic conditions, newly diagnosed issues, and prescribed medications
  • Identify and address any care gaps by offering actionable recommendations
  • Focus solely on the evaluation processthere is no requirement to prescribe or develop treatment plans

Qualifications:

  • Active Nurse Practitioner license in Florida
  • Board Certification (AANP or ANCC)
  • Current CPR certification (BLS)
  • MSN from an accredited nursing program
  • At least 6 months of recent NP experience is preferred
