Embrace an impactful per diem opportunity as a Nurse Practitioner delivering essential home health risk assessments across Volusia County. This role specializes in evaluating Medicare Advantage plan members in their own residences, offering both autonomy and the chance to make a tangible difference in patient outcomes. The typical home health risk assessment visit lasts about 45 minutes and may cover annual assessments or post-discharge check-ins.
Schedule:
- Flexible schedulingchoose your own shifts between Monday and Sunday
- Set your preferred weekly or bi-weekly hour commitments
- Assignments may include Deltona, Daytona Beach, Port Orange, or nearby areas within Volusia County
Key Responsibilities:
- Complete comprehensive annual home health risk assessments for Medicare Advantage plan members
- Assess complete medical histories, current conditions, and medication usage
- Educate individuals on their chronic conditions, newly diagnosed issues, and prescribed medications
- Identify and address any care gaps by offering actionable recommendations
- Focus solely on the evaluation processthere is no requirement to prescribe or develop treatment plans
Qualifications:
- Active Nurse Practitioner license in Florida
- Board Certification (AANP or ANCC)
- Current CPR certification (BLS)
- MSN from an accredited nursing program
- At least 6 months of recent NP experience is preferred
