Deliver compassionate, proactive care as a Nurse Practitioner specializing in home health risk assessments for Medicare Advantage plan members. Collaborate with a respected healthcare organization serving residents across Fulton County, including areas such as Wauseon, Archbold, and Swanton. Schedule:

• Flexible shifts available between Monday and Sunday
• Design your own schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform comprehensive annual and post-discharge home health risk assessments on Medicare Advantage members
• Review patients past and current medical histories, existing conditions, and medication regimens
• Educate individuals on the management of their health conditions and medications
• Identify care gaps and suggest recommendations to improve patient outcomes
• Focus solely on assessmentno requirement to create treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in Ohio
• Current CPR certification (BLS)
• Board certification from AANP or ANCC
• MSN degree from an accredited nursing program
• Minimum of 6 months NP experience preferred
