Take on a rewarding per diem Nurse Practitioner opportunity focused on home health risk assessments for Medicare Advantage plan members. As part of your daily responsibilities, you will conduct thorough assessments in various locations across Delta County, including Delta, Orchard City, and Paonia.
Schedule:
- Flexible shifts available any day of the week, Monday through Sunday
- Set your own hours with a minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform comprehensive annual home health risk assessments for Medicare Advantage members in their homes
- Review and evaluate both previous and current medical history, conditions, and medication usage
- Provide patient education regarding medical conditions and medications
- Identify care gaps and recommend next steps to help resolve them
- No requirement to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Colorado
- Board certification (AANP or ANCC)
- Current BLS/CPR certification
- MSN degree from an accredited nursing program
- At least 6 months of recent NP experience (preferred)
