A per diem opportunity is available for an experienced Nurse Practitioner to perform home health risk assessments in Allen County, Ohio. This role involves traveling to members residences in a variety of communities, including Lima and surrounding townships, to evaluate Medicare Advantage plan members. You will make a vital difference for patients by assessing health status and engaging in meaningful patient educationall without the responsibility for prescribing medications or developing treatment plans.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule
- Weekly or bi-weekly minimum hourly commitment
Key Responsibilities:
- Complete in-home health risk assessments, lasting approximately 45 minutes per visit
- Review and evaluate current and past medical history, ongoing conditions, and medication use
- Deliver clear patient education regarding medical issues and medications
- Identify gaps in care, offering actionable recommendations for resolution
- Visits may include annual health assessments as well as post-discharge follow-ups
Qualifications:
- Active and unrestricted Nurse Practitioner license in Ohio
- Board-certified through AANP or ANCC
- Minimum of a Master of Science in Nursing (MSN) from an accredited institution
- Current CPR (BLS) certification required
- At least 6 months of nurse practitioner experience, preferably in a home health environment
