Job Description
An excellent opportunity is available for a Nurse Practitioner experienced in home health to conduct home health risk assessments for Medicare Advantage plan members throughout White County, IN. This per diem position is ideal for providers seeking flexibility, autonomy, and a rewarding experience supporting patient health in the home setting. Locations include Monticello, Brookston, Monon, and surrounding communities.
Schedule:
- Flexible shifts offered Monday through Sunday
- Providers may create their own schedules
- Minimum weekly or bi-weekly hourly commitments available
Key Responsibilities:
- Perform comprehensive annual health risk assessments in patients homes, focusing on existing medical history, current conditions, and medication review
- Complete post-discharge and annual wellness assessments where applicable
- Educate patients on chronic and acute medical conditions, as well as medication regimens
- Identify care gaps and provide recommendations for further medical attention
- No treatment planning or prescribing required
Qualifications:
- Active Nurse Practitioner license in Indiana
- Board Certification through AANP or ANCC
- MSN degree from an accredited nursing school
- Current CPR (BLS) certification
- At least 6 months of recent experience working as a Nurse Practitioner is preferred
In this role, evaluations are expected to last approximately 45 minutes, and nurses are compensated $80-$90 per evaluation, with an additional mileage stipend for travel and compensation for administrative time.
