Make a meaningful impact by delivering in-home healthcare risk assessments as a Nurse Practitioner. This per diem opportunity allows you to provide critical patient evaluations for Medicare Advantage members throughout Volusia County, including areas such as Deltona, Daytona Beach, and Port Orange. Each assessment typically takes about 45 minutes and may include either annual wellness visits or post-discharge evaluations.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own schedule
- Minimum weekly or bi-weekly hourly commitments to suit your needs
Key Responsibilities:
- Perform comprehensive home health risk assessments for Medicare Advantage plan members
- Review and document detailed medical, surgical, and medication histories
- Educate patients on their health conditions and medication management
- Identify and address gaps in care by providing relevant recommendations
- No responsibility for developing treatment plans or prescribing medications required
Qualifications:
- Current Nurse Practitioner license in Florida
- Active BLS/CPR certification
- National board certification (AANP or ANCC)
- At least 6 months of recent Nurse Practitioner experience preferred
- Master of Science in Nursing (MSN) from an accredited institution
This position offers $80$90 per evaluation, along with a mileage stipend and compensation for administrative time. You'll benefit from a flexible schedule, allowing you to balance this role with other professional or personal commitments.
