As a Nurse Practitioner specializing in home health assessments, youll make a meaningful impact on patient care in a flexible, per-diem role. This position involves conducting home health risk assessments for Medicare Advantage plan members throughout Benton County, including areas such as Warsaw, Cole Camp, Lincoln, and White Branch. Each risk assessment takes about 45 minutes and may be focused on an annual health check or follow-up after hospital discharge. Enjoy the autonomy of creating your own schedule while delivering essential health evaluations directly in patients homes.

Schedule:

• Flexible, self-scheduled shifts available Monday through Sunday
• Set your minimum weekly or bi-weekly commitment to match your preferences

Key Responsibilities:

• Perform thorough home health risk assessments for Medicare Advantage members
• Review prior and current medical histories, conditions, and medication regimens
• Educate patients on their medical conditions and proper medication use
• Identify gaps in care and suggest actionable recommendations
• No treatment planning or medication prescribing required

Qualifications:

• Active Nurse Practitioner license in Missouri
• Board certification via AANP or ANCC
• Current CPR certification (BLS required)
• MSN degree from an accredited nursing school
• At least 6 months of recent NP experience preferred
