This Nurse Practitioner role focuses on providing evaluations for Medicare Advantage plan members in areas including Rupert, Heyburn, and Paul. Schedule:

• Flexible shifts available Monday through Sunday
• Design your own work schedule
• Meet minimum weekly or bi-weekly hourly commitments

Key Responsibilities:

• Conduct home health risk assessments for Medicare Advantage members
• Review and evaluate previous and current medical history, medications, and ongoing conditions
• Educate patients about their health status, medical conditions, and medications
• Identify care gaps and provide recommendations for improvement
• No requirement for treatment planning or prescribing medications

Desired Qualifications:

• Active Nurse Practitioner license in Idaho
• Current BLS/CPR certification
• Board Certified through AANP or ANCC
• Minimum of six months experience practicing as a Nurse Practitioner (preferred)
• MSN degree from an accredited nursing program
