This Nurse Practitioner role focuses on providing evaluations for Medicare Advantage plan members in areas including Rupert, Heyburn, and Paul. Assessments average 45 minutes each and may involve annual health checkups or post-discharge visits. Enjoy autonomy and a schedule that adapts to your life while positively impacting patient care.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule
- Meet minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
- Conduct home health risk assessments for Medicare Advantage members
- Review and evaluate previous and current medical history, medications, and ongoing conditions
- Educate patients about their health status, medical conditions, and medications
- Identify care gaps and provide recommendations for improvement
- No requirement for treatment planning or prescribing medications
Desired Qualifications:
- Active Nurse Practitioner license in Idaho
- Current BLS/CPR certification
- Board Certified through AANP or ANCC
- Minimum of six months experience practicing as a Nurse Practitioner (preferred)
- MSN degree from an accredited nursing program
