Job Description
Deliver vital care directly to patients in their homes by conducting home health risk assessments for Medicare Advantage plan members throughout Cassia County, including Burley, Oakley, and Declo. This per diem opportunity is ideal for Nurse Practitioners with a passion for personalized healthcare and flexible scheduling.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule to fit your needs
- Choose your preferred hourly commitment on a weekly or bi-weekly basis
Key Responsibilities:
- Perform comprehensive annual home health risk assessments for Medicare Advantage members
- Review and evaluate patients past and current medical histories, conditions, and medications
- Educate patients on their health conditions and prescribed medications
- Identify potential gaps in care, and provide actionable recommendations for improvement
- Visits average approximately 45 minutes each; responsibilities do not include creating treatment plans or prescribing medications
Required Qualifications:
- Active Nurse Practitioner License in Idaho
- Board Certification through AANP or ANCC
- Current CPR (BLS) certification
- MSN degree from an accredited nursing institution
- Minimum of 6 months of current NP experience preferred
