Step into a vital community-based care role as a Nurse Practitioner delivering home health risk assessments within Franklin County. This per diem opportunity empowers you to shape your own schedule while providing critical healthcare evaluations to Medicare Advantage members in areas such as Winchester, Decherd, and Estill Springs.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own schedule to fit your lifestyle
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Conduct comprehensive annual or post-discharge health risk assessments in patients homes
- Review and evaluate prior and current medical history, chronic conditions, and medication regimens
- Educate members about medical conditions and prescribed or over-the-counter medications
- Identify gaps in care and offer practical recommendations for resolution
- No requirement for creating treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in Tennessee
- Current certification in CPR (BLS)
- Board certification from either AANP or ANCC
- At least six months of recent Nurse Practitioner experience preferred
- Master of Science in Nursing (MSN) from an accredited institution
